Patient care begins with an evaluation.

We first assess your range of motion, pain level, strength and ability to perform functional tasks.

Our skilled therapists will establish a treatment plan designed specifically for you. The focus is on relieving symptoms and setting individualized goals to help you recover in the shortest amount of time possible.
We ask that you arrive at least 15 minutes before your scheduled appointment. When you arrive, you’ll be required to fill out patient forms, or you can print and fill them out here prior to your appointment.

Please be prepared for your visit:

  • Dress appropriately:
  • Wear comfortable shoes
    Nonrestrictive, closed-toe shoes preferred
  • Wear comfortable clothes
    We suggest clothing that allows exposure to the hurt area (for example, if you have a knee injury, you may wear shorts)
  • We provide patient gowns, if necessary
  • Bring:
  • Prescription for physical therapy
  • Current insurance card
  • Photo identification
    (driver’s license, passport, military identification or student identification)
  • List of your current medications
  • Complete your patient forms:
You can complete these patient forms in advance, or complete the copy provided by one of our Patient Service Representatives at check-in.
